Sustainability and Transformation Partnership

Since the end of 2016, Isle of Wight Clinical Commissioning Group has been working with 20 health and care organisations across Hampshire and the Isle of Wight to determine how best to meet the many opportunities and challenges facing the local health and care system. This has focused on both the need to empower people to stay well and also on how we can provide safe, high quality, consistent and affordable health and care to everyone.

Since the Sustainability and Transformation Partnership delivery plan was developed we have had a number of achievements, across the STP area, including:

  • Training 4,000 staff to ‘Make Every Contact Count’
  • Developing plans to introduce integrated primary care hubs, including successfully starting two
  • Carrying out significant work to ensure all patients have a detailed hospital discharge plan
  • Moving inpatient vascular services to University Hospital Southampton to ensure local people have the best clinical outcome possible
  • Carrying out an extensive engagement programme with patients, their carers and staff to understand the needs of people who have severe mental illness.
