Long Terms Conditions

The IWCCG Long Term Conditions team intend to improve the quality of life for people suffering from long term conditions (LTC) such as hypertension, diabetes, heart and respiratory conditions, supporting them to manage their conditions and to avoid complications which require hospital treatment.

There is significant evidence of what works to support people with a LTC - this includes early intervention, self help, integrating service provision and making sure services are delivered locally.

Our services include Rehabilitation, Community Matrons, Podiatry, Orthotics and also Specialist Nurses for Parkinson’s Disease, Stroke, Multiple Sclerosis, Respiratory problems and Heart Failure to name just a few.

We work very closely with Age UK, Action on Hearing Loss, the Isle of Wight Blind Society, Headway, Community Action (IOW) and many other local and national charities as well as the Local Authority and St Mary’s and mainland Hospitals.

We produce a Long Term Conditions Newsletter and host a Long Term Conditions Patient Group, to which anyone with a LTC can belong.
