Frail Elderly Services

The Frail Older Persons agenda focuses on improving care for people aged 75 years or older (but not restricted to the over 75s) with a combined identification of fragility. Through partnership working (between the NHS, the council, voluntary services, care homes etc) services are being developed to improve the quality of care provided and to prevent people from deteriorating unnecessarily. This includes:

  • Identifying people early and providing early coordinated care and support
  • Developing care plans led by the patient (which aren’t duplicated unnecessarily), which identify care needs and peoples wishes
  • Providing more services in the Community, including a Consultant Geriatrician and a crisis response service
  • Falls prevention services
  • The use of assisting technology, to support people and their carers to address challenges to everyday living and enhance their independence (monitored by professionals).
