Continuing Healthcare

The NHS Funded Care Team is responsible for managing an annual budget for NHS Continuing Healthcare services in the region of £10m and is accountable for:

  • Securing through commissioning effective, appropriate and high quality health services in line with individual’s health needs and in line with the National Guidance. Such services include care home placements including those with nursing care, and domiliciary care packages at home, as well as NHS services such as GP input, community and specialist nurses and occupational therapy, physiotherapy, speech and language services etc.
  • Procurement based on partnership and multi-agency working which requires a productive working relationship with colleagues across health, Local Authority, voluntary and independent sectors, and includes safeguarding vulnerable adults.
